We're looking for a PRN Hospice Chaplain who is passionate about providing support and grief counseling!

Qualifications and Skills:

• Bachelor's degree in theology or related degree.
• Experience providing spiritual, grief, and loss support preferred
• Excellent communication, organizational, and interpersonal skills.

Responsibilities/essential functions:

Must be able to perform the following essential job functions with or without reasonable accommodations.

• Assess the patient for spiritual needs and implement interventions based on patient goals.
• Completes a comprehensive assessment and plan of care within 5 days of admission.
• Facilitates visits by local clergy, pastoral, or other local faith leaders as requested by patients.
• Educates the patient and family on hospice services and the role of spiritual support..
• May conduct religious services of prayer and worship at the request of patients and families.
• May participate in providing bereavement services.
• Participates in the Quality Assurance Performance Improvement Program.
• Documents at point of care; documentation is accurate and reflects patient decline.
• May provide spiritual support to hospice employees and other community members when applicable.
• Completes education, participates in education in-services, continuing education, seminars, and self-study.
• Maintains compliance with policies, procedures, State and Federal regulations, and accreditation standards.
